Willow Creek Assisted Living & Memory Care at San Martin offers a wide selection of resident services. Willow Creek is a fully licensed Assisted Living Community.

Basic Services offered to all Willow Creek residents include:

• 24-hour staff
• Three meals daily plus snacks
• Social and community services
• All utilities (except telephone)
• Community Health Resource referrals
• Weekly housekeeping
• Weekly linen service
• Apartment maintenance
• Scheduled transportation
• Planned activity schedule

Residents can start out with basic resident services and add higher levels of assistance as they become desirable. Assisted Living & Memory Care services are tailored to the requirements of the individual resident.

Respite Care and Adult Day Care are also available.

Willow Creek Caregivers work closely with the residents' physician to comply with their Plan of Care and with the families to ensure satisfaction.
